Nominate a Mental Health Professional

The Association is now accepting nominations for the 2009 Mental Health Professional Award of Excellence.

Please include in the following in your nomination:

• Name and contact information for nominee

• Your name and contact information

• Description of how this person has made a positive difference in the mental health of yourself or someone you know

• Why you think this mental health professional deserves special recognition for their efforts and what makes the work of this mental health professional an example of excellence

The deadline for nominations has been extended to June 5th, 2009.

Be as specific and descriptive as possible in your nomination. You may nominate more than one person but each should be submitted on a separate nomination form. Forms may be handwritten or typed.

Nominations will be reviewed by a member of the staff of the Mental Health Association in Greensboro, a person with experience with mental health issues, and a mental health provider. The award will be announced and presented at our end of year celebration on June 16th, 2009.
